Challenges are community funding resources

I tend to prepare the Vets during meetings so they are well aware of the transition or discharge

Although CTI is a great model, many of our potential referring agents like VA staff from VA programs don't understand that CTI is not traditional case management. They are trying to refer Vets who have failed multiple programs due to declining cognition, acute substance abuse issues and really need more intensive case management - how do other grantees "sell" this service.
